Office Policy

Financial Policy
At Mountain Gate Dental payment is expected at time of service. We bill dental insurance as a courtesy, and also require your estimated patient portion at time of service. If a payment plan is needed, an agreement will be made with a credit card on file to be charged by Mountain Gate Dental. We accept several forms of payment for dental treatment provided Cash, Check, Debit Card, Credit Card and Care Credit are accepted as forms of payment. Terms and Conditions may apply.

Dental Insurance:
Understanding your insurance coverage can be quite a challenge. Our goal at Mountain Gate Dental is to assist you in utilizing your benefits; however, each plan is different in its coverage. It is important for you to understand that in most cases your insurance is designed to reduce your cost, NOT eliminate it completely. We encourage you to become familiar with your policy exclusions, deductibles and required copayments.

Our courtesy service to you includes:
We will make every effort to inform you of your treatment needs and associated fee ranges. We follow the American Dental Association guidelines for coding procedures.

We will file your insurance electronically (when available) and request payment be sent directly to us. Any payment not received after sixty (60) days of filing becomes the responsibility of the patient. Payment from the patient is expected within ten (10) days of notification, unless prior written arrangements are made.

Our expectations of you as the owner of the policy:
Payment for services not covered by your insurance plan are due at time of treatment, this will include any copayments or deductibles.

Please understand that the insurance policy belongs to you and we have no leverage to obtain payment from your insurance carrier. Realize that dental insurance policies restrict payment for some services, use restricted fee schedules (called UCR) and exclude some procedures based on prior conditions or length of time on the plan.

It will be your responsibility to pay any fees your insurance has not covered after 60 days. If any amount is left outstanding, subsequent statements will be charged a $3.00 late fee per month of the outstanding balance.

Mountain Gate Dental Missed or Cancelled Appointment Policy
Mountain Gate Dental requires at least 24 hour notice for any changes to your appointment. We understand that schedules change and emergencies arise. If you do not give at least 24 hour notice of any changes to your appointment, there may be a $50 charge to your account. If you are more than 10 minutes late to your appointment, you will be marked as a missed appointment. Multiple missed appointments could result in termination as a valued patient from Mountain Gate Dental.

Financial Options

Dr. Erekson and our team at Mountain Gate Dental never want our patients to be concerned about treatment cost and avoiding what your smile needs to last for years to come. We offer several payment options you and your family’s dental care.

We are a preferred provider for most major insurance companies. We suggest calling our office before your visit to confirm your coverage, and our team members are always happy to help you file your claim and review your coverage. We want you to receive the most out of your dental visits! If you do not have dental insurance or if you need to cover copayments or out-of-pocket expenses, our office accepts the following forms of payment:

  • Cash

  • Check

  • Credit cards

In addition to the above payment methods, we have other options for patients without dental insurance, and we have partnered with CareCredit® and LendingClub. These are third party lenders that may be able to finance the entire cost of your treatments, often with low or no down payments or interest rates.
